Physical options:
If you would like for our school physician (Dr. Paul Hamm, D.O.) to complete the physical, please complete the CIPPE packet and turn it into the school office, no later than May 19th. Physicals will be completed in the afternoon on May 21st and May 28th. The cost of the physical is $15.00 (cash or check made payable to NCHS).
When completing the CIPPE packet, please make sure to complete all sections and be sure to:
Complete Section 1: fill out the entire page with all information requested. Include your medical insurance carrier and policy number.
Complete Sections 3-6: please make sure that you and your athlete read through, sign and date each section in the appropriate areas.
If you elect to have your family healthcare provider or an urgent care provider complete the physical, please submit the physical to the school office prior to August 1, 2025 (football participants may be required to be submitted earlier). Physicals must be completed after May 1, 2025 to be valid for the upcoming school year athletic season.
The CIPPE packet is available on the NCHS website https://north-clarion.schoolblocks.com under the “Athletics” section or in the school office.
*Fall athletics include football, cross country, golf, Jr. varsity & varsity volleyball, and soccer. There will be opportunities for school-provided physicals for additional sports throughout the school year.
